TM-1 application that briefly describes the nature of the submission and what code sections or governing documents the application has been filed to comply with

PW-1 application (if applicable)

Site drawings, floor drawings, building drawings, sprinkler drawings, engineering drawings signed and sealed by design professional (if applicable)

Narrative describing the scope of work (if applicable)

Manufacturer’s specification of equipment and manual (if applicable)

Engineering or hazard analysis signed and sealed by design professional (if applicable)

Material Data Safety Sheet (MSDS) (if applicable)

Calculations (if applicable)

Test reports from nationally recognized and accredited laboratory (if applicable)

Simulation files:

• If Fire Dynamics Simulator (FDS) was used, please submit all input and output files for the conducted simulation.
• If another nationally recognized Computational Fluid Dynamics (CFD) program was used, please submit videos of the conducted simulation showing as much of the structure in question or subject of analysis as possible.

For an amendment not previously filed on FDNY Business, you can access previous FDNY records by providing your reference account number (FPIMS #) and premises zip code. It is highly recommended that you link your previous record, which can be done at the beginning of the application.
