Before you submit your application

    The applicant must submit a current for-hire Insurance Certificate (FH-1). Insurance must be issued in the applicant's name. Applicant can get a for-hire Insurance Certificate (FH-1) directly from his or her insurance carrier.

      The applicant must submit current DMV registration, Certificate of Title, Bill of Sale, or Leasing Agreement. *NOTE: Vehicles licensed out of state must provide DMV registration showing seating capacity.

        Government issued photo I.D. of applicant. State issued driver's license preferred.

          A valid and original Social Security Card.

            The applicant must have a N.Y.S. Department of Transportation Inspection form (MC300) showing vehicle has been inspected. The front and back page (defect report) of the document is required. Defect report must be provided if vehicle inspection report indicates denied. (The inspection report is valid for one (1) year, and it must be current at time of application). To arrange an inspection, the applicant must contact the nearest DOT regional office.               The applicant must submit proof of business status. If the owners are a corporation, the applicant must produce a Certificate of Incorporation and a filing receipt. If a partnership, the applicant must produce a Partnership Certificate. If a sole proprietor, the applicant must produce a Business Certificate. To get original copies of a Certificate of Incorporation, Filing Receipt or Business Certificate contact the NYS Division of Corporations.

                The applicant must submit an IRS issued CP-575 Notice or a 145-C letter (provided as a replacement or EIN verification letter to business,) social security cards & a government issued photo I.D. for each elected officer. Please see the Contact local IRS office.

                  Fees must be submitted along with the application form, as identified below. All fees are non-refundable.

                    The license application, required documents, and fees must be filed in person at a TLC Licensing center from Monday through Friday between the hours of 8:00am and 4:00pm.

                      The New Vehicle Application fee is $550.
